Intradialytic hypotension frequency is reduced by levocarnitine supplementation

چکیده

Introduction: Intradialytic hypotension (IDH) is a frequent complication of hemodialysis. IDH causes intradialytic discomfort and subclinical ischemia, resulting in higher rate morbidity mortality. Levocarnitine (LC) administration has been suggested for the treatment IDH, but conflicting reports about its efficacy have published. We describe effect LC supplementation patients experiencing recurrent episodes, spite common strategies used to prevent it. Methods: Sixteen hemodialysis were studied. was defined as drop systolic blood pressure ≥20 mmHg, with or without symptoms, prompting an intervention by dialysis staff, such reducing/stopping ultrafiltration and/or fluid administration. Blood recorded 192 sessions, before supplementation. Thereafter (30 mg/kg dry weight) administered at beginning each hemodialysis, registering 384 treatments. The difference between predialysis minimum evaluated (∆SBP), after Cardiac ejection fraction also measured. Results: Predialysis postdialysis systolic, diastolic, mean arterial pressures did not differ Before supplementation, 36 episodes occurred (19%), while 29 during sessions (8%; χ2 = 16.03; p 0.0001). ∆SBP lower even though significant (p 0.22). Conclusion: frequency significantly reduced which can be helpful patients’ well-being reduction IDH-associated risks.

Intradialytic hypotension strikes again.

Intradialytic hypotension (IDH) has plagued hemodialysis (HD) patients and their caregivers since the early days of renal replacement therapy. Its persistence in the current era is evident from the results of the 1426-patient Hemodialysis (HEMO) study in this issue of JASN in which a median of 12.5% of dialysis sessions were complicated by IDH requiring saline infusion, lowering the ultrafiltra...

Intradialytic hypotension and vascular access thrombosis.

Identifying potential modifiable risk factors to reduce the incidence of vascular access thrombosis in hemodialysis could reduce considerable morbidity and health care costs. We analyzed data from a subset of 1426 HEMO study subjects to determine whether more frequent intradialytic hypotension and/or lower predialysis systolic BP were associated with higher rates of vascular access thrombosis. ...

Use of fludrocortisone for intradialytic hypotension

Intradialytic hypotension during dialysis adversely affects a patient's prognosis and increases mortality. We report a case in which intradialytic hypotension that persisted after the administration of midodrine was relieved after the use of fludrocortisone. Administration of 0.2 mg of fludrocortisone occurred 30 minutes before dialysis. We compared 45 sessions of dialysis without fludrocortiso...

Intradialytic hypotension and risk of cardiovascular disease.

BACKGROUND AND OBJECTIVES Patients undergoing hemodialysis have an elevated risk of cardiovascular disease-related morbidity and mortality compared with the general population. Intradialytic hypotension (IDH) is estimated to occur during 20%-30% of hemodialysis sessions. To date, no large studies have examined whether IDH is associated with cardiovascular outcomes. This study determined the pre...

Intradialytic hypotension: frequency, sources of variation and correlation with clinical outcome.

Intradialytic hypotension (IH) is a frequent complication of hemodialysis (HD) and is associated with increased patient mortality and cardiovascular events. We studied IH to determine its variability, correlates, and clinical impact in 13 outpatient HD facilities. Blood pressure was captured by machine download. IH was defined as >30 mmHg decrease in systolic blood pressure to <90 mmHg. Risk fa...
